COVID-19 omicron variant outbreak in a hematopoietic stem cell transplant unit
Int J Hematol. 2023 Aug 2. doi: 10.1007/s12185-023-03638-3. Online ahead of print.ABSTRACTRecommendations and guidelines for management of SARS-COV-2 infection in hematologic patients were developed in the very difficult context of dealing with novel viral variants from one pandemic wave to another, with different susceptibility to available drugs and vaccines. Moreover, the largest SARS-COV-2 case series in patients treated for hematologic malignancies, including stem cell transplant recipients, was published before the Omicron surge, and refers mainly to Alpha and Delta viral variants. These infections had very high mort...

Vaccination of allogeneic haematopoietic stem cell transplant recipients: Guidelines from the Francophone Society of Bone Marrow Transplantation and Cellular Therapy (SFGM-TC)
Bull Cancer. 2023 Jul 19:S0007-4551(23)00289-8. doi: 10.1016/j.bulcan.2023.05.007. Online ahead of print.ABSTRACTDuring immune reconstitution following allogeneic haematopoietic stem cell transplantation (allo-HSCT), (re)vaccination of allo-HSCT recipients is recommended. Herein, we propose an update of practical recommendations regarding vaccination of allo-HSCT recipients. These recommendations, based on data from the literature, national and international guidelines and the consensus of the participants when no formally proven data are available, were elaborated during the workshop of practice harmonization organized by...

Varicella vaccine meningoencephalitis in a child receiving autologous bone marrow transplantation
CONCLUSIONS: Our experience highlights the importance of conducting a thorough medical history regarding vaccination and serological status of newly immunocompromised patients. Intensive chemotherapy succeeding live vaccine administration <4 weeks could have influenced early and severe viral reactivation. Early initiation of prophylactic antiviral treatment is questioned in such circumstances.PMID:37395442 | DOI:10.1111/petr.14562

Steroid induced secondary immune deficiency
Ann Allergy Asthma Immunol. 2023 Jan 18:S1081-1206(23)00011-X. doi: 10.1016/j.anai.2023.01.010. Online ahead of print.ABSTRACTDespite their widespread clinical use, oral corticosteroids (OCSs) are well known to be associated with a myriad of adverse effects, including immunosuppression. By inhibiting transcription factors and affecting leukocyte function, prolonged OCS use leads to significant CD4 lymphopenia, and also often leads to a decrease in serum IgG. Conversely, OCS use has minimal impact on circulating B cell, serum IgM, or serum IgA levels. Although there is a paucity of literature, individuals treated with prolo...
